Healthy Memphis Common Table
 

Data Center

is a collaborative effort of the University of Tennessee, University of Memphis and the Healthy Memphis Common Table (HMCT) to support health and healthcare improvement initiatives throughout the Mid-South, by using hard data to track our community’s progress toward measurable health goals.

 

Mission

  • To create a health and healthcare data infrastructure to:
  • Support health promotion efforts for children and women in the Mid-South,
  • Support Initiatives of the Healthy Memphis Common Table and its Community Partners and
  • Assist in community-wide health and healthcare data reporting.

Key Long-term Goals

  • Measure area provider and community success in providing STEEEP (safe, timely, effective, efficient, equitable and patient-centered) prevention-focused care.
  • Obtain, tract and report short-term, intermediate & long-term outcome measures for local health & healthcare initiatives.
  • Obtain, track and report behavioral risk factor data related to obesity and diabetes.
  • Make the Healthy Memphis Data Center a resource for the entire Memphis and Shelby County community, and a model for the state of Tennessee.

  • Provide scientific evidence regarding best practices and models for HMCT.
  • Assist in overall process and outcomes evaluation of the HMCT Obesity/Diabetes initiative
  • Help HMCT Community Partner Working Groups/Action Teams and Project Teams:
    • Establish measurable outcome goals,
    • Develop data collection strategies,
    • Monitor and document progress toward goals,
    • Document outcomes of 2005 intervention Projects
