Key Activities
Healthy Memphis assists its Community Partners and the people of the Mid-South through bringing partners together & being the Mid-South catalyst for health and health care improvement.
The goal of this Activity is to develop “Action Plans” or “Call to Action” initiatives to give organizations networking opportunities in the context of a strategic planning process devoted to identification of best opportunities for effective concerted actions and a proven peer group implementation support process. The HMCT plan is to focus on specific health issues, set achievable goals and common strategies that are community wide and community driven to achieve measurable improvements in the health of the average Memphian. HMCT’s goal is for this activity to maintain active involvement and provide significant value to our major Community Partners.
These meetings will serve to update Community Partners and the general public regarding ongoing HMCT Community Partner initiatives and progress toward achieving measurable community health and healthcare goals. In addition, they will serve to help identify and prioritize key community health and healthcare needs and best methods for addressing these needs.
Once a year, it is anticipated that the Quarterly Community Partner Meeting will serve as a HMCT Summit that includes the usual quarterly meeting activities, but goes farther to shine a spotlight on key community health needs, key actions needed, and community progress in addressing community health and health care needs.
Emails are sent regularly to update Community Partners about ongoing activities of the HMCT and its Community Partners. Newsletters also share reports on best evidence and models related to the key initiatives of the HMCT.
An educational resource for Community Partners to help them:
- Find best evidence regarding types of interventions that are effective in addressing critical community health needs,
- Find best models or practices employed by other organizations or communities or organizations that have been shown to be effective, and
- Find other community partners with whom they can collaborate to improve the health or healthcare of their employees or constituents.
This website will also serve as a educational resource for the people of the Mid-South to help them:
- Identify their own key health and healthcare needs,
- Take charge of their own health, healthcare and healthcare resources.
In addition it will serve as a vehicle for all Community Partners to make their services and resources available to healthcare consumers in the greater Memphis area.
