Become a Community Partner  Community Partners can participate in all of the activities of the Healthy Memphis Common Table.  Community Partners are individuals, coalitions or organizations who commits great effort, skill, or resources to improve the health of people in the Mid-South.

COMMUNITY PARTNER COMMITMENT

The undersigned individual, organization or coalition hereby pledges to work with common purpose to promote the health and abundant life of our friends and neighbors, to improve and to coordinate our community’s health system, to encourage public accountability for our health and health care resources, and to work together, both providers and consumers of health care, as Community Partners of the Healthy Memphis Common Table. 

As a Community Partner, the undersigned indicates their commitment to:

1. The HealthCARE principles of the Healthy Memphis Common Table

2. Active and regular participation in at least one HMCT collaborative Working Group per year.

3. Annual contribution of funds, resources, time OR talent, in proportion to the Community Partner’s means or ability, in order to support the work of the Healthy Memphis Common Table.
